Short answer: Oral-B’s round, oscillating head cleans tooth-by-tooth and is easier to steer around brackets, crowns and crowded teeth. Sonicare’s sonic head sweeps a whole section at once, feels gentler, and runs roughly twice as long per charge. If your mouth has hardware in it — braces, implants, bridges, tight spacing — or you want the option of a small round head, buy Oral-B. If you have receding gums, sensitive teeth, or you brush hard and want the most forgiving option, buy Sonicare. Neither brand is meaningfully “better” at cleaning teeth.

Two things in this comparison are not what the internet says they are, and both change the answer. The widely-quoted clinical figures for “Oral-B beats sonic” are the wrong numbers from the wrong study. And the folklore that “Oral-B brush heads are cheaper” stopped being true when Oral-B moved to the iO system — which is the only Oral-B line that actually competes with a premium Sonicare. Below is the full arithmetic.

The one real difference: round head vs sonic sweep

Oral-B uses a small round head that oscillates and rotates, cleaning one tooth at a time. Sonicare uses an oval head that vibrates side to side at high frequency, cleaning a section at a time and pushing fluid between teeth. That single mechanical choice drives every other difference between the brands — the feel, the technique, the noise, the battery life and the running cost.

The Oral-B round head is roughly the diameter of a tooth. You hold it on each tooth for a couple of seconds and move on. It feels busy and mechanical, it is louder, and it gives obvious tactile feedback. Because the head is small and rigid, it reaches places a larger oval head has to approach at an angle — behind lower front teeth, around a bracket, along the edge of a crown.

Sonicare’s head covers two to three teeth. You place it along the gumline at a slight angle and glide slowly. Philips’ 2026 motor runs at a stated 62,000 bristle movements per minute, which registers as a high-frequency buzz rather than a scrub. Most people find it markedly more comfortable, and people who dislike electric brushes usually dislike the oscillating sensation specifically.

Using Oral-B technique on a Sonicare — pressing and holding tooth by tooth — is the single most common reason someone says a new Sonicare “does not clean as well.” It cleans fine. It is being driven wrong.

Which brand suits your mouth?

This is the actual decision, and it is not “which brand wins.” It is which mechanism suits what is already in your mouth. Six situations cover almost everyone.

You have receding gums or exposed root surfaces

Sonicare, with the intensity turned down. Exposed root is dentine, not enamel, and it is softer and more sensitive. A sonic head sweeping across the gumline distributes force over a wider area than a rotating head pressed onto one spot, and every current Sonicare above the entry model offers reduced intensity levels rather than only alternate modes. The DiamondClean 9000 is the most adjustable of them. If recession is active or new, that is a conversation for your dentist before it is a shopping decision.

You have sensitive teeth

Sonicare, again, but check what you are actually buying. “Sensitive mode” on many brushes just shortens the cycle or changes the pulse pattern; what helps is genuinely lower power. Sonicare models with three intensity levels let you drop the power on every mode. Oral-B’s answer is its Sensitive and Gum Care modes plus a softer head, which works, but the mechanism is still oscillation against a sensitive surface. If a manual brush already makes you wince, start with sonic.

You have braces, brackets or a permanent retainer

Oral-B, clearly. A small round head can be walked around an individual bracket and under an archwire in a way an oval head cannot, and Oral-B sells a dedicated Ortho head shaped for exactly that. This is the one situation where the brands are not close. The iO Series 2 is enough — you are buying the head shape, not the electronics, and its pressure sensor slows the motor automatically, which matters when you are working around hardware. Orthodontic patients should also treat the brush as an addition to interdental brushes and floss threaders, not a replacement for them.

You have crowns, implants, bridges or veneers

Either, but ask your dentist first, and buy the pressure sensor whichever way you go. The risk with restorations is not the brush type, it is force at the margin where the restoration meets the tooth. Both brands offer pressure sensors; Oral-B’s implementation on the iO line physically reduces motor speed as well as lighting up, which is the more useful behaviour. Do not buy a model without one, and do not use a “whitening” or polishing head on veneers or composite unless your dentist says to.

You are buying for a child

Oral-B for the head size, Sonicare if the noise is the problem. A small round head is easier for a child to control and easier for a parent to supervise, because you can see exactly which tooth it is on. The counter-argument is real, though: some children refuse an oscillating brush outright and accept a sonic one, and a brush that gets used beats a brush that is theoretically better. Both brands sell dedicated kids’ ranges with softer bristles and lower power — do not simply hand a child an adult flagship on its lowest setting.

You travel constantly, or you hate charging things

Sonicare, and it is not close. Philips states 21 days of runtime for its current 5900, 6500 and 7300 models; Oral-B’s iO flagships are rated around two weeks and independent reviewers commonly report ten to fourteen days in practice. For a two-week trip that is the difference between packing a charger and not. Sonicare also uses a standard click-on head that any Sonicare handle in a hotel drawer or a partner’s bathroom will accept.

This is your first electric toothbrush

Buy the cheapest model from either brand that has a pressure sensor and a two-minute timer, and spend nothing more. The jump from a manual brush to any powered brush is far larger than the jump between a $60 brush and a $300 one. Buy at the bottom, live with it for six months, and you will then know from your own mouth whether you want the other mechanism — which is worth more than any comparison article, including this one.

Runtimes are manufacturer figures; independent reviewers generally find Sonicare meets its claim and Oral-B lands at the lower end of “about two weeks.”

What five years actually costs — with the arithmetic

Both brands say to replace the head every three months, so four heads a year, twenty heads over five years. That consumable is where the real money is: on every tier here, five years of heads costs more than the brush did. Here is the arithmetic, using per-head price bands observed in August 2026.

Three conclusions fall straight out of that table, and two of them contradict what almost every other comparison says.

One: at genuine-head prices the two brands are a wash. Buy in sensible multipacks and five years of Oral-B iO heads runs roughly $160–200 against roughly $120–160 for Sonicare. That is a difference of perhaps $40 over five years — about eight dollars a year, or less than the price swing on a single Black Friday deal. Nobody should choose a mechanism over eight dollars a year.

Two: the “Oral-B heads are cheaper” claim is real, but it is about the wrong Oral-B. The figure that circulates — Oral-B saving you $100–150 over five years — comes from comparing Oral-B’s classic round heads at roughly $5–8 against Sonicare’s genuine heads. Those classic heads fit the old push-on Oral-B handles. They do not fit an iO. If you buy an iO because you read that Oral-B is cheaper to run, you have bought into the one Oral-B line where that is no longer true.

Three: Sonicare has the lower floor, by a lot. Because Sonicare’s click-on fitting is a simple mechanical mount that has not changed in years, there is a large third-party head market, and those heads run one to three dollars. Whether you use them is a judgement call — fit is usually slightly looser and bristle quality is inconsistent — but the option exists, and it takes the five-year consumable bill down to roughly $30–60. There is no equivalent option on iO, for a reason worth its own section.

The brush-head lock-in nobody mentions

Oral-B runs two brush-head ecosystems that are not compatible with each other, and Sonicare runs one. This is the most consequential practical difference between the brands and it is almost never mentioned in a comparison.

Every Oral-B iO handle, from the iO2 to the iO10, uses a magnetic drive. Power is transferred without a physical shaft, and the head contains a small chip that talks to the handle. No classic Oral-B head — CrossAction, Precision Clean, FlossAction, Sensitive — will fit an iO handle, and there is effectively no third-party iO head market, because a generic part has to replicate both the magnetic coupling and the electronics.

Sonicare, by contrast, has kept the same click-on mount across its range for over a decade. An A3 Premium All-in-One head fits a DiamondClean 9000, a new 7300, an old ProtectiveClean, and almost anything else with the click-on fitting. You can buy whichever genuine head is cheapest that week regardless of which model you own, mix head types in a household that owns several handles, and use third-party heads if you want to.

What this means in practice: a Sonicare purchase leaves your future options open, and an iO purchase closes them for as long as you own the handle. If you are the sort of person who hunts for deals on consumables, that asymmetry is worth more than any feature on the spec sheet. If you buy heads once a year without thinking about it, it costs you nothing. Decide which of those you are before you spend $200.

What the clinical evidence actually says

The headline figures you have seen quoted — that oscillating-rotating brushes remove 11% more plaque and reduce gingivitis 6% more than sonic — are the wrong numbers. Those are from the 2014 Cochrane review of powered versus manual toothbrushes (Yaacob and colleagues, 56 studies, 5,068 participants), which found powered brushes of all kinds reduced plaque by 11% and gingivitis by 6% at one to three months, rising to 21% and 11% beyond three months. That review compares electric against manual. It does not compare Oral-B against Sonicare.

The Cochrane review that does compare powered types against each other is a separate one, on different powered toothbrushes for plaque control and gingival health. Across seven trials of up to three months, it found rotation-oscillation brushes reduced plaque more than side-to-side brushes with a standardised mean difference of 0.24 (95% CI 0.02 to 0.46), and reduced gingivitis with an SMD of 0.35 (95% CI −0.04 to 0.74).

Two things about those numbers matter more than their direction. The plaque confidence interval only just clears zero. The gingivitis interval crosses zero, which means that result did not reach statistical significance. And the review’s authors state plainly that the difference is small, that its clinical importance is unclear, and that the underlying trials were at unclear or high risk of bias.

The honest summary: published evidence gives Oral-B’s mechanism a small measured edge on plaque, an unproven one on gum health, and both from a thin, low-confidence evidence base. What is much better established is that any powered brush beats a manual one, and that how long and how gently you brush swamps the brand difference entirely. A Sonicare used for two full minutes twice a day will beat an Oral-B used for forty seconds every time, and no study contradicts that.

Again: this is a summary of published research for the purpose of comparing two products. It is not clinical advice, and it does not account for anything specific about your mouth.

Which features are worth paying for

Worth it: the pressure sensor. This is the only feature on either brand with a clear mechanical rationale — brushing too hard is a genuine cause of gum and enamel wear, and almost everyone does it without knowing. Prefer an implementation that changes the brush’s behaviour rather than only lighting up; Oral-B’s iO sensors slow the motor, which is harder to ignore than a light you cannot see with the brush in your mouth. Do not buy any electric toothbrush at any price without one.

Worth it: a two-minute timer with quadrant pacing. Free on essentially everything, and it fixes the most common failure in real brushing, which is stopping early.

Usually not worth it: app connectivity and position detection. Both brands sell tracking that maps which surfaces you missed. It is genuinely clever, and the honest observation is that almost nobody keeps using it — the phone has to be out, propped up and unlocked at 6:50am, every day, forever. Treat the app as a two-week training aid you might use once, not as a reason to spend a hundred dollars more. If tracking is the only difference between two models, buy the cheaper one.

Usually not worth it: extra modes. Beyond Clean, Sensitive and Gum Care, the additional modes on a twelve-setting brush are combinations of power level and duration. A brush with three intensity levels gives you more useful control than one with seven named modes.

Situational: the travel case. A plain case is worth having; a USB-charging case is worth real money only if you travel monthly — and on a Sonicare with 21 days of runtime it is largely redundant.

Ignore entirely: whitening claims. Whitening modes and heads remove surface stain more thoroughly, which is what any brush does given adequate time. Neither changes the intrinsic colour of a tooth.

Model-number traps in both ranges

Philips reused the number 4100 on a slower brush, and this is the single easiest way to buy the wrong thing in 2026. The old ProtectiveClean 4100 (model HX6810) ran at 62,000 brush movements per minute. The current 4100 Series (model HX3681) is a different, entry-level product running at roughly 31,000. Same headline number, roughly half the speed. Reviews and “best of” lists written before 2026 that praise “the Sonicare 4100” are usually praising the discontinued one. Check the HX model number, not the marketing name.

More broadly, Philips retired the ProtectiveClean naming for a numeric ladder — 4100, 5000, 5700, 5900, 6000, 6500, 7100, 7300 — and only the 5700 and above carry the re-engineered 2026 motor Philips describes as its first major drivetrain change in over a decade. If you want the new motor, 5700 is the floor.

On the Oral-B side the trap is the word “Limited.” Oral-B sells many retailer-specific iO bundles — “iO Series 3 Limited Deep Clean,” “iO5 Limited Deep Clean & Whiten” — that differ from the standard model mainly in which heads and case are in the box. They are usually fine and often cheaper; just compare what is in the carton rather than assuming a longer name means a better brush.

And on both sides, ignore the percentages. “700% more plaque removal,” “1000%,” “2000%” are all measured against a manual toothbrush under manufacturer test conditions, not against each other. A 2000% brush is not twice as good as a 1000% brush. They are marketing units and they compare nothing you care about.

Buy now or wait?

Electric toothbrushes are one of the most heavily discounted categories in consumer retail, and the swing is large enough that timing genuinely matters. Both brands routinely run 40–50% off flagship models during major sale events, so the same brush can be a poor buy in September and a good one in October. If you can wait a few weeks, wait.

The calendar that moves this category: Amazon’s autumn Prime event in early-to-mid October, Black Friday and Cyber Monday at the end of November, and the New Year health-and-wellness reset in early January. Both brands treat these as their heavy promotional periods, and the deepest discounts land on last season’s flagship rather than the newest model.

What does not move much: brush heads. Consumables discount shallowly and irregularly, which argues for buying a large genuine multipack whenever you see one rather than waiting for an event. Note also the new-model dynamic on the Philips side — with the 2026 range now shipping, remaining DiamondClean 9000 and older ProtectiveClean stock is being cleared, which is where the sharpest Sonicare pricing currently sits.

Buy now anyway if your current brush has died, your dentist has told you to change something, or you are still using a manual brush. The gain from brushing better starting this week is worth more than a percentage off in two months.

Buy neither, if…

Buy neither of these if what you actually want is a brush that runs for two minutes and stops you pressing too hard. That is the great majority of the benefit and it is available for well under fifty dollars. Everything above that price buys modes, screens, apps and battery life — not cleaning. Skip this whole comparison if:

  • You have never used an electric brush. Start cheap, learn which sensation you tolerate, then upgrade with real information about your own mouth.
  • You are buying for a household of three or four. Four premium handles is a serious amount of money for a marginal gain. Four budget brushes that everyone actually uses is the better outcome.
  • You are hard on your possessions or travel with checked baggage. A $300 brush is a $300 loss.
  • Your real problem is between the teeth. No powered brush of any price reaches interdental surfaces properly. Floss, interdental brushes or a water flosser will do more for you than upgrading from a $60 brush to a $250 one.

The iO Series 6 is the sensible stopping point; the iO Series 2 gets you the same head shape for much less. Accept that you are locked into iO-only heads for the life of the handle.

Buy Sonicare if you have sensitive teeth or receding gums, if you travel, or if you want to keep your consumable options open. The 5900 Series is the value pick in the new 2026 range; the DiamondClean 9000 is the one to buy if comfort is the problem you are solving.

And if neither of those paragraphs describes you, that is a real answer too: buy the cheapest brush from either brand that has a pressure sensor, and put the saved money into flossing. The evidence supports that more strongly than it supports either brand over the other.
